Insurance Information

Most services at CRG are considered mental health services, which are often covered differently from medical care. We recommend contacting your insurance provider before your first appointment to understand your specific benefits.

In-Network vs. Out-of-Network
 

CRG is in-network with Sagamore Health Network

All other insurance providers are considered out-of-network

Sagamore patients:

Responsible for copays, deductibles, and coinsurance

CRG submits claims to your insurance

Out-of-network patients:

Payment is due at the time of service

CRG submits claims as a courtesy

Reimbursement (if applicable) is sent directly to you

Important Things to Check with Your Insurance
 

Before your visit, we recommend confirming:
 

Mental health coverage and benefits
 

Whether your provider type is covered (e.g., psychologist, psychiatrist)
 

Whether your specific service is covered (e.g., therapy vs. testing)
 

If prior authorization is required
 

Any diagnosis limitations or exclusions
 

Whether multiple services in one day are covered

Good Faith Estimate

In compliance with the No Surprises Act, which went into effect on January 1, 2022, all healthcare providers must notify clients of their federal rights and protections against “surprise billing.”

This Act requires that we, Children’s Resource Group/CRG, notify our patients of their federally protected rights to receive a notification when services are rendered by an out-of-network provider, if a client is uninsured, or if a client elects not to use their insurance.

In addition, we provide our patients with a Good Faith Estimate of the cost of services. Below is a fee schedule for each provider and the services they typically offer.
