Prescription Refill Requests
If you need a refill, please call our 24-hour refill line and leave a detailed message.

Please include:
Your name (and patient name, if different)
Daytime phone number
Medication name and dosage
Provider’s name
Date of your next appointment
Additional Details:
Indicate if you prefer pickup or mailing
If sending to a pharmacy, include pharmacy name + phone number
Allow 2 days for pickup or 5 days for mailed prescriptions.
Letter Requests
If you need documentation from your provider (e.g., school, work, or other purposes), please complete the Provider Letter Request Form.
What to Expect:
Your provider will review and determine any applicable fees
Typical cost: $55–$275, depending on time required
Processing time: up to 2 weeks
Medical Records Requests
To request records, please complete the appropriate form:
The Release of Information indicates the information you would like us to send to a designated third party, such as another doctor’s office, school, or organization.
If you would like a copy of your chart for your own personal records, please complete the Request to Access PHI Form.
What to Expect:
Requests are processed within up to 30 days
Records must be reviewed and approved by your provider
Expedited (“rush”) requests are limited
Fees:
Small records (under 20 pages): Low or no cost
Larger records: $5–$50, depending on size
Additional fees may apply for:
Postage
Certified mail ($20)
Rush processing ($10)

Cancellation Policy
If you need to cancel or reschedule an appointment, please provide at least 24 hours’ notice.
Missed appointments or late cancellations (less than 24 hours) will be charged the full appointment fee
Evaluation Policy
Comprehensive evaluations at CRG follow a structured, multi-step process designed to provide clear and accurate results.
Evaluations typically include:
Initial Visit
A detailed appointment to review concerns, history, and background information.
Testing Appointment
Standardized testing to assess cognitive, academic, and emotional functioning.
Follow-Up Appointment
Review of results, diagnoses (if applicable), and recommendations.
Written Report
A comprehensive summary with findings and next steps.
Additional Details
Testing sessions may last several hours and are typically scheduled in the morning
Breaks are provided during testing
A detailed report is shared after the follow-up appointment
Cost & Payment
Payment is due at the time services are provided
Evaluation fees vary based on complexity and time required
Costs are typically split between testing and follow-up/reporting
Payment Responsibility
While CRG assists with insurance coordination, the responsibility for payment remains with the patient or the financially responsible party.
